Introducción: La COVID-19 significó un gran reto para los servicios de cirugía a nivel mundial, lo que trajo como consecuencia modificaciones, incluso la suspensión de la actividad quirúrgica en algunos casos con el objetivo de garantizar seguridad tanto para el paciente como para el personal de salud. Objetivo: Describir acciones que contribuyan a garantizar condiciones de bioseguridad en ambientes quirúrgicos durante la pandemia de COVID-19. Métodos: Se realizó una revisión bibliográfica para la cual se usaron 37 referencias bibliográficas en inglés y español. Se consultaron fuentes científicas como PubMed/Medline, SciELO, Scopus, ScienceDirect y fuentes oficiales como la Organización Mundial de la Salud. Desarrollo: Ante la reanudación de la cirugía electiva se crearon protocolos de actuación. Deben clasificarse los pacientes en 3 grupos según la posibilidad de padecer COVID-19: individuos sanos, portadores asintomáticos y pacientes con síntomas. Además, debe darse prioridad a los pacientes cuyos procedimientos fueron cancelados. Se preconiza crear circuitos independientes y separados para evitar el contacto de casos sospechosos o confirmados de COVID-19 con el resto de pacientes. Para garantizar mayor seguridad debe entrenarse al personal médico en la forma correcta de utilizar los medios de protección personal. La inducción anestésica de estos pacientes debe garantizar su seguridad y prevenir el contagio. Una vez concluida la cirugía, se llevará a cabo la recuperación inicial del paciente dentro del propio quirófano y este será higienizado estrictamente. Conclusiones: Los profesionales sanitarios deben estar adecuadamente entrenados y conocer las medidas de bioseguridad y protocolos sanitarios tanto del hospital como del país en que se encuentren.


Introduction: COVID-19 posed a great challenge to surgical services worldwide, resulting in modifications, including the suspension of surgical activity in some cases with the aim of ensuring safety for both the patient and the health personnel. Objective: To describe actions that contribute to guarantee biosafety conditions in surgical environments during the COVID-19 pandemic. Methods: A bibliographic review was carried out using 37 bibliographic references in English and Spanish. Scientific sources, such as PubMed/Medline, SciELO, Scopus and ScienceDirect, were consulted; as well as official sources, such as the World Health Organization. Development: When elective surgery was resumed, action protocols were created. Patients should be classified into 3 groups, according to the possibility of having COVID-19: healthy individuals, asymptomatic carriers, and patients with symptoms. In addition, priority should be given to patients whose procedures have been cancelled. Independent and isolated wards are recommended to be created in order to avoid contact between suspected or confirmed COVID-19 cases and the rest of the patients. To ensure greater safety, the medical personnel should be trained in the correct use of personal protective equipment. The anesthetic induction of these patients should ensure their safety and prevent contagion. Once the surgery is over, the initial recovery of the patient will be carried out inside the operating room itself, which shall be strictly sanitized. Conclusions: Healthcare professionals must be adequately trained and be aware of the biosecurity measures and healthcare protocols of both the hospital and the country where they are located.

We present a computational model for electrochemical surface-enhanced Raman scattering (EC-SERS). The surface excess of charge induced by the electrode potential (Vel) was introduced by applying an external electric field to a set of clusters [Agn]q with (n, q) of (19, ±1) or (20, 0) on which a molecule adsorbs. Using DFT/TD-DFT calculations, these metal-molecule complexes were classified by the adsorbate partial charge, and the main Vel-dependent properties were simultaneously studied with the aid of vibronic resonance Raman computations, namely, changes on the vibrational wavenumbers, relative intensities, and enhancement factors (EFs) for all SERS mechanisms: chemical or nonresonant, and resonance Raman with bright states of the adsorbate, charge-transfer (CT) states, and plasmon-like excitations on the metal cluster. We selected two molecules to test our model, pyridine, for which Vel has a remarkable effect, and 9,10-bis((E)-2-(pyridin-4-yl)vinyl)anthracene, which is almost insensitive to the applied bias. The results nicely reproduced most of the experimental observations, while the limitations of our approach were critically evaluated. We detected that accounting explicitly for the surface charges is key for EC-SERS models and that the highest calculated EFs, up to 107 to 108, are obtained by interstate coupling of bright local excitations of the metal cluster and CT states. These results highlight the importance of nonadiabatic effects in SERS and the capabilities of EC-SERS as a technique with potential to study excited-state coupling by tuning the CT and plasmon-like states by manipulating Vel.

OBJECTIVE: The objective of this study was to determine cut-off points that can be used to differentiate measures of empathy, which would then be classified as high, medium, or low. To do so, we used data from students from 7 medical schools in Colombia, El Salvador, and the Dominican Republic, after determining the psychometric properties of the 3-dimensional model of empathy in the Jefferson Scale of Empathy, S-version (for medical students). MATERIALS AND METHODS: This non-experimental descriptive study had a sample that consisted of 6291 students. The structure and factor invariance were analyzed by country and sex. A hierarchical cluster analysis and a bifactorial analysis of variance were applied. RESULTS: The measure of empathy was reliable on the global scale (α = .82; ω = .88). A confirmatory factor analysis showed that the original model was replicable and adjusted to the data (comparative fit index [CFI] = .90; goodness of fit index = .94), while the multigroup analysis allowed to assume an invariant factor structure by country and gender (ΔCFI < .01). Tables were constructed with cut off points for empathy and its dimensions. DISCUSSION AND CONCLUSION: Our study solves the problem of comparing the scores and the levels of empathy observed in the medical students at different schools of medicine, making said comparisons within and between countries and between genders. The instrument used has adequate psychometric properties and the cut-off values obtained allow the classifying of people with lower or higher levels of empathy.

Introducción: La cirugía torácica videoasistida por un solo puerto (Uniportal VATS) se ha convertido hoy en uno de los abordajes más utilizados para resecciones pulmonares en muchos centros de cirugía torácica del mundo, en Cuba su introducción es bastante reciente y todavía se encuentra en fase inicial. Objetivo: Comparar los resultados obtenidos por cirugía torácica abierta y Uniportal VATS en pacientes intervenidos en el Instituto Nacional de Oncología y Radiobiología de La Habana (INOR) durante enero de 2016 a abril de 2017. Material y Métodos: Estudio observacional descriptivo de corte longitudinal retrospectivo con 60 pacientes atendidos en el INOR durante enero de 2016 a abril de 2017 a los cuales se les realizó cirugía torácica. Resultados: Veinticuatro pacientes fueron operados por Uniportal VATS y 36 a través de la toracotomía, primaron las lesiones del lado derecho, el procedimiento más realizado fue lobectomía. Los estadíos predominantes fueron IA y IB, la mayoría de los procedimientos fue realizada en tres horas o menos, 8 casos se convirtieron, 13 sufrieron complicaciones. La estadía media de los pacientes tratados con Uniportal VATS fue menor que los que recibieron la toracotomía abierta. Conclusiones: El abordaje Uniportal se caracterizó por ser seguro, tener una gran versatilidad, pocas complicaciones y por mejorar la recuperación postquirúrgica del paciente y acelerar su reincorporación a las actividades de la vida cotidiana(AU)


Introduction: Single-port Video Assisted Thoracic Surgery (Uniportal VATS) has currently become one of the most commonly used approaches for lung resections in many Thoracic Surgery Centers around the world. In Cuba, its introduction is quite recent and it is still in its initial phase. Objective: To compare the results obtained by open thoracic surgery and Uniportal VATS in patients who underwent surgery in the National Institute of Oncology and Radiobiology of Havana (INOR) from January 2016 to April 2017. Material and Methods: An observational descriptive longitudinal retrospective study was conducted in 60 patients who underwent Thoracic Surgery in the INOR during 2016 - April 2017. Results: A total of 24 patients were operated through Uniportal VATS and 36 underwent thoracotomy. The lesions predominantly affected the right side and lobectomy was the most performed surgical procedure. Stages IA and IB predominated. Most of the procedures were carried out in 3 hours or less, 8 cases were converted and 13 suffered from complications. The average postoperative length of stay in hospital for patients who underwent Uniportal VATS was reduced compared to those who underwent open thoracotomy. Conclusions: Uniportal VATS is characterized by being a safe procedure with few complications and great versatility. It improves postoperative recovery of patients and accelerates their return to daily life activities(AU)

Silver nanoparticles functionalized with thiolated ß-cyclodextrin (CD-SH) were employed for the detection of bisphenols (BPs) A, B, and S by means of surface-enhanced Raman spectroscopy (SERS). The functionalization of Ag nanoparticles with CD-SH leads to an improvement of the sensitivity of the implemented SERS nanosensor. Using a multivariate analysis of the SERS data, the limit of detection of these compounds was estimated at about 10-7 M, in the range of the tens of ppb. Structural analysis of the CD-SH/BP complex was performed by density functional theory (DFT) calculations. Theoretical results allowed the assignment of key structural vibrational bands related to ring breathing motions and the inter-ring vibrations and pointed out an external interaction due to four hydrogen bonds between the hydroxyl groups of BP and CD located at the external top of the CD cone. DFT calculations allowed also checking the interaction energies of the different molecular species on the Ag surface and testing the effect of the presence of CD-SH on the BPs' affinity. These findings were in agreement with the experimental evidences that there is not an actual inclusion of BP inside the CD cavity. The SERS sensor and the analysis procedure of data based on partial least square regression proposed here were tested in a real sample consisting of the detection of BPs in milk extracts to validate the detection performance of the SERS sensor.

ABSTRACT Objective: To estimate the levels of empathy in students enrolled in the nursing program. Material and methods. Data Sources: The population consists of students from the first to the fifth academic year (Course) (N = 312, n = 253, 81.08% of the population). Selection of studies: Exploratory and cross-sectional study. Data extraction: The authors collected the information between January and February 2018. Participants answered the Jefferson Medical Scale of Empathy, the Spanish version for medical students (S version), validated and adapted for nursing students in Colombia. Results. Synthesis of the data: Cronbach's alpha was satisfactory (not typifed = 0.891 and typified = 0.886), from which it is inferred that the data has internal reliability. The interclass correlation coefficient was 0.891; CI [0.87, 0.909] (F = 9.15, p = 0.005): confirms the good reliability. The authors observed that in empathy, this coefficient was highly significant in all three factors, with the eta-square value satisfactory and a high power observed. The significance level used was a < 0.05 and P <0.20. Conclusions: The manifestation of the levels of empathy observed in this study agrees with the concept of empathic erosion, especially when analyzing empathic behavior in the male gender. The empathy levels in the students examined are relatively low, and authors conclude that there is considerable potential for growth of empathy and that of its components. Empathy is a complex part of academic formation that cannot be seen as an isolated variable.


RESUMEN Objetivo: Estimar los niveles de empatia en estudiantes matriculados en un programa de enfermería. Material y Métodos. Fuentes de datos: La población está compuesta por estudiantes del primer al quinto año académico (Curso) (N = 312, n = 253, 81.08% de la población). Selección de estudios: Estudio exploratorio y transversal. Extracción de datos: La información se recopiló entre enero y febrero de 2018. Los participantes respondieron la Escala de Empatía Médica de Jefferson (S)validada y adaptada para estudiantes de enfermería en Colombia. Resultados. Síntesis de los datos: El alfa de Cronbach fue satisfactorio (no tipificado = 0,891 y tipificado = 0,886): de manera que los datos tienen fiabilidad interna. El coeficiente de correlación interclase fue de 0,891; IC [0.87, 0.909] (F = 9.15, p = 0.005): este confirma la buena confiabilidad. Se observó que, en la empatía, el coeficiente de correlación intraclase fue altamente significativo en los tres factores; con el valor eta-cuadrado satisfactorio y la potencia observada alta. El nivel de significación utilizado fue a < 0.05 y P <0.20. Conclusiones: La manifestación de los niveles de empatía observados en este estudio coincide con el concepto de erosión empática, especialmente cuando se analiza el comportamiento empático en el género masculino. En general, los niveles de empatía en los estudiantes examinados son relativamente bajos y se ha demostrado que existe un potencial considerable para el crecimiento de la empatía y de sus componentes. La empatía es una parte compleja de la formación académica que no puede verse como una variable aislada.

Surface-enhanced Raman scattering of thiobenzoic acid and thiobenzamide have been recorded on three different silver colloids in order to find the chemical species responsible for the spectra and to detect differences in the adsorption with respect to their oxygen counterparts, benzoic acid and benzamide, respectively. Very significant and unexpected shifts of opposite sign between the Raman and SERS wavenumbers have been detected. By comparing the experimental and DFT calculated wavenumbers, it can be concluded that the acid is bonded to the metal as thiobenzoate through the sulfur atom with unidentate coordination. SERS spectra of thiobenzamide can be explained by assuming that it is adsorbed as azanion, like in the case of benzamide, linking to the metal through the sulfur and nitrogen atoms of the ionized thiocarboxamide group. In order to support these conclusions, B3LYP/LanL2DZ force field calculations for different complexes of silver cations with the thiobenzoate anion, the neutral thiobenzamide as well as its azanion have been carried out. Additionally, the 8a;νring mode is the most enhanced band in the SERS of both adsorbates pointing to the participation of a metal-to-molecule resonant charge transfer mechanism.

The detection of emerging contaminants in the aquatic environment, such as ibuprofen and caffeine, was studied by means of surface-enhanced Raman spectroscopy (SERS) using Ag nanoparticles (AgNPs) synthesized with ß-cyclodextrin (ßCD) as a reducing agent. The effect on the SERS signal of different molar ratios of Ag+/ßCD in the synthesis route and the aging process of AgNPs were investigated by using trans-cinnamic as a test molecule. The SERS effectiveness of these ß-cyclodextrin colloids (Ag@ßCD) was also checked and compared with that of other silver sols usually employed in SERS synthesized by using other reducing agents such as citrate, borohydride and hydroxylamine. All the synthesized SERS substrates were characterized by different techniques. The experimental results indicate that Ag@ßCD with the more diluted Ag+/ßCD molar ratio showed the best SERS signal, enabling detection at trace concentrations of 0.5 µM in the case of trans-cinnamic acid. The Ag@ßCD sols also showed the best sensitivity for detecting ibuprofen and caffeine, reaching the lowest limit of detection (0.1 mM). The proposed synthetic route for Ag@ßCD sols provides an improved SERS substrate for detecting organic pollutants with better performance than other standard silver sols.

EL HOMA-IR (homeostasis model assessment-insulin-resistance) es un estimador de insulinorresistencia (IR) pero depende de la determinación de insulina. Los índices triglicéridos-glucosa (T-G)-circunferencia de la cintura (CC) (T-G-CC) o triglicéridos-glucosa-índice de masa corporal (TG- IMC) podrían ser sustitutos. Los objetivos de este trabajo consistieron en investigar en personas con riesgo de desarrollar diabetes tipo 2 (DT2): a) los índices T-G, T-G-CC y T-G-IMC como estimadores de HOMA-IR>2,1; b) determinar su poder discriminante. Se realizó un estudio prospectivo en el que se estudiaron 223 individuos ≥45 años con riesgo de desarrollar diabetes tipo 2 (DT2). La relación T-G se calculó como ln [triglicéridos (mg/dL) x glucemia (mg/dL)/2]. La relación T-G-CC y T-G-IMC fue el producto de T-G por CC o IMC. Se utilizó análisis de regresión logística y se calcularon las áreas bajo las curvas ROC (receiver operating characteristic curves) (ABC) para comparar las asociaciones de T-G, T-G-CC y T-G-IMC con HOMA-IR>2,1. Mediante análisis discriminante se evaluó la clasificación de los sujetos entre HOMA-IR>2,1 y HOMA-IR≤2,1. ABC, sensibilidad, especificidad, poder predictivo positivo y negativo para T-G-CC y T-G-IMC fueron mayores que para T-G, con los siguientes valores de corte: T-G=8,75, T-G-CC=821 y T-G-IMC=255. Los odds ratios (OR) para HOMA-IR>2,1, ajustados para confusores, fueron: T-G>8,75, OR: 4,85 (IC 95% 2,73-8,62); T-G-CC>821, OR: 10,41 (IC 95% 5,55-19,53); T-GIMC> 255, OR: 10,41 (IC 95% 5,55-19,53). Con el análisis discriminante T-G>8,75 clasificó correctamente 69,2% individuos con HOMA-IR≤2,1 y 68,3% con HOMA-IR>2,1; T-G-CC y T-G-IMC clasificaron 74,4% y 78,2% respectivamente (p<0,001 en todos los casos). Se concluyó que T-GCC> 821 y T-G-IMC>255 fueron mejores estimadores de HOMA-IR>2,1 que T-G>8,75. Estas son determinaciones simples y accesibles y podrían ser útiles en la práctica clínica y en estudios epidemiológicos.


HOMA-IR ((homeostasis model assessment-insulin-resistance) is a surrogate estimator of insulin resistance (IR) but it depends on insulin determination. Triglyceride-glucose-waist circumference (T-G-WC) or triglyceride-glucose-body mass index (BMI) (T-G-BMI) could be substitutes. The objectives of this work were: to investigate in people at risk of developing type 2 diabetes (T2D): a) T-G, T-G-CC and T-G-BMI as estimators of HOMA-IR>2.1 and b) to determine their discriminating power. A prospective study was conducted studying 223 individuals ≥45 years of age at risk of developing type 2 diabetes (T2D). The T-G ratio was calculated as ln [triglycerides (mg/dL) x glycemia (mg/dL)/2]. The T-G-CC and T-G-BMI ratio was the product of T-G by CC or BMI. Logistic regression analysis was used and the areas under the receiver operating characteristic curves (ROC) curves were calculated to compare the associations of T-G, T-G-CC and T-G-BMI with HOMA-IR>2.1. Using a discriminant analysis, the classification of the subjects between HOMA-IR>2.1 or HOMA-IR≤2.1 was evaluated. AUC, sensitivity, specificity, positive and negative predictive powers for T-G-CC and T-G-BMI were higher than for T-G, with the following cut-off values: TG=8.75, T-G-CC=821 and T-G-BMI=255. Odds ratios (OR) for HOMA-IR>2.1, adjusted for confounders, were: T-G>8.75, OR 4.85 (95% CI 2.73-8.62); T-G-CC>821, OR 10.41 (95% CI 5.55-19.53); T-G-BMI>255, OR 10.41 (95% CI 5.55-19.53). With the discriminant analysis T-G>8.75, 69.2% correctly classified with HOMA-IR≤2.1 and 68.3% with HOMA-IR>2.1; T-G-CC and T-G-BMI correctly classified 74.4% and 78.2% respectively (p <0.001 in all cases). It is concluded that T-G-CC>821 and T-G-BMI>255 were better estimators of HOMA-IR>2.1 than T-G>8.75. T-G-WC and T-G-BMI are simple and reliable determinations and could be useful in clinical practice and epidemiological studies.

A major challenge of tuberculosis diagnosis is the lack of universal accessibility to bacteriological confirmation. Computer-aided diagnostic interventions have been developed to address this gap and their successful implementation depends on many health systems factors. A socio-technical system to implement a computer-aided diagnostic tuberculosis diagnosis was preliminary tested in five primary health centers located in Lima, Peru. We recruited nurses (n = 7) and tuberculosis physicians (n = 5) from these health centers to participate in a field trial of an mHealth tool (eRx X-ray diagnostic app). From September 2018 to February 2019, the nurses uploaded images of chest X-rays using smartphones and the physicians reviewed those images on web-based platforms using tablets. Both completed weekly written feedback about their experience. Each nurse participated for a median duration of 12 weeks (interquartile range = 7.5-15.5), but image upload was only possible at a median of 58 percent (interquartile range = 35.1%-84.4%) of those weeks. Each physician participated for a median duration of 17 weeks (interquartile range = 12-17), but X-ray image review was only possible at a median of 52 percent (interquartile range = 49.7%-57.4%) of those weeks. Heavy workload was most frequently provided as the reason for missing data. Several infrastructural and technological challenges impaired the effective implementation of the mHealth tool, irrespective of its diagnostic accuracy.

RESUMEN Introducción: El mesotelioma pleural maligno es un tumor maligno primario de la pleura, comúnmente asociado con la exposición al asbesto. Se considera una patología rara y muy agresiva. Objetivo: Realizar una revisión sobre los criterios de diagnóstico y tratamiento actualizados en torno al mesotelioma pleural maligno. Métodos: Se realizó una revisión bibliográfica en fuentes de información disponibles en la Biblioteca Virtual de Salud, de la red telemática Infomed, entre ellas, las bases de datos SciELO, Pubmed/Medline, Cumed, Lilacs, así como el Google Académico. Se seleccionaron un total de 39 referencias. Conclusiones: Existen pocas referencias en la literatura nacional relacionadas con el diagnóstico, tratamiento y seguimiento de los pacientes con mesotelioma pleural maligno. El diagnóstico combina el uso del método clínico, los estudio imagenológicos e histoquímicos. No existe un tratamiento estándar, siendo recomendable un enfoque individualizado que combine según cada caso, cirugía, quimio y radioterapia. Los desafíos futuros incluyen el desarrollo de nuevas alternativas terapéuticas(AU)


ABSTRACT Introduction: Malignant Pleural Mesothelioma is a primary malignant tumor of the pleura, commonly associated with exposure to asbestos. It is considered a rare and very aggressive pathology. Objective: Conduct a review of updated diagnostic and treatment criteria for malignant pleural mesothelioma. Material and Methods: A bibliographic review was made through the search of information in sources available from the Cuban National Health Care Network and Portal (INFOMED), among them, databases such as SciELO, Pubmed / Medline, Cumed, Lilacs, as well as Google Scholar. Finally, a total of 39 references were selected for our study. Conclusions: There are few references in the national literature related to the diagnosis, treatment and follow-up of patients with malignant pleural mesothelioma. The diagnosis combines the use of the clinical method, the imaging and histochemical studies. There is no standard treatment, being recommended an individualized approach that combines according to each case, surgery, chemo and radiotherapy. Future challenges include the development of new therapeutic alternatives(AU)

Introduction: Streptococcus agalactiae is the main etiological agent causing invasive infection of the newborn with symptoms that may be associated with septicemia, pneumonia or meningitis and prevalences up to 50% worldwide where there is an increase in antibiotic resistance. Objective: To estimate the prevalence of vagino-rectal colonization by S. agalactiae and its sensitivity profile in pregnant women attending a third-level hospital. Materials and methods: One hundred and twenty one pregnant women were sampled by vaginal and rectal swabs. The cultures were carried out following the methodology recommended by the CDC, and chromID Strepto B agar was added. The suggestive colonies were identified biochemically and the sensitivity profiles according to CLSI were determined. As control, S. pneumoniae ATCC 49619 and S. agalactiae ATCC 12403 were used. Results: Pregnant colonization prevalence by S. agalactiae was 20.66%, with a total of 40 isolates of which 12.5% were non-sensitivity to penicillin. Sensitivity to levofloxacin, clindamycin and erythromycin was 100%, 92.5% y 87.5%, respectively, with the phenotypes iMLSB (3/40) and M (2/40). No sensitivity to tetracycline was found. Conclusions: The prevalence of vagino-rectal colonization by S. agalactiae in the study population was 20.66%, obtaining isolates not sensitive to penicillin and resistance to macrolides and lincosamidas by the Kirby-Bauer technique, so the importance of carrying out active active in pregnant women colonized by Streptococcus agalactiae and to perform constant epidemiological surveillance to detect changes in the sensitivity profiles of the isolates.

Las colonias sugestivas se identificaron bioquímicamente y se determinaron los perfiles de sensibilidad según el Clinical and Laboratory Standards Institute (CLSI). Resultados. La prevalencia de colonización por S. agalactiae en las mujeres gestantes fue del 20,66 %. Se obtuvieron 40 aislamientos del total de muestras analizadas, de los cuales, el 12,5 % no presentó sensibilidad a la penicilina. La sensibilidad a la levofloxacina, la clindamicina y la eritromicina fue de 100 %, 92,5 % y 87,5 %, respectivamente; no se encontró sensibilidad a la tetraciclina. El fenotipo iMLSB se encontró en tres y, el M, en dos de los 40 aislamientos. Conclusiones. La prevalencia de colonización vaginorrectal por S. agalactiae en la población de estudio, fue de 20,66 %. Se obtuvieron aislamientos no sensibles a la penicilina, y con resistencia a los macrólidos y las lincosamidas mediante el método de Kirby-Bauer. Por ello, es importante la búsqueda activa en las mujeres gestantes colonizadas por estreptococos del grupo B y la vigilancia epidemiológica constante para detectar cambios en los perfiles de sensibilidad de los aislamientos.

Electrochemical surface-enhanced Raman scattering (SERS) of the cruciform system 1,4-bis((E)-2-(pyridin-4-yl)vinyl)naphthalene (bpyvn) was recorded on nanostructured silver surfaces at different electrode potentials by using excitation laser lines of 785 and 514.5 nm. SERS relative intensities were analyzed on the basis of the resonance Raman vibronic theory with the help of DFT calculations. The comparison between the experimental and the computed resonance Raman spectra calculated for the first five electronic states of the Ag2-bpyvn surface complex model points out that the selective enhancement of the SERS band recorded at about 1600 cm-1, under 785 nm excitation, is due to a resonant Raman process involving a photoexcited metal-to-molecule charge transfer state of the complex, while the enhancement of the 1570 cm-1 band using 514.5 nm excitation is due to an intramolecular π→π* electronic transition localized in the naphthalenyl framework, resulting in a case of surface-enhanced resonance Raman spectrum (SERRS). Thus, the enhancement of the SERS bands of bpyvn is controlled by a general chemical enhancement mechanism in which different resonance processes of the overall electronic structure of the metal-molecule system are involved.

A Surface-Enhanced Raman Scattering (SERS) spectrum of 4-cyanopyridine (4CNPy) was recorded on silver plasmonic nanoparticles and analyzed by using Density Functional Theory (DFT) calculations. Two simple molecular models of the metal-4CNPy surface complex with a single silver cation or with a neutral dimer (Ag+-4CNPy, Ag2-4CNPy), linked through the two possible interacting sites of 4CNPy (aromatic nitrogen, N, and nitrile group, CN), were considered. The calculated vibrational wavenumbers and intensities of the adsorbate and the isolated species are compared with the experimental Raman and SERS results. The analysis of the DFT predictions and the experimental data indicates that 4CNPy adsorbs preferentially on neutral/charged active sites of the silver nanoparticles through the nitrogen atom of the aromatic ring with a perpendicular orientation.

BACKGROUND: The clinical value of sentinel lymph node biopsy (SLNB) in patients with thick melanoma is uncertain. The purpose of this study was to investigate the correlations between survival and lymph node status in thick melanomas. METHODS: Of a total of 736 melanoma patients registered between 2000 and 2016, 50 presented with thick melanomas (≥ 4.0 mm) without distant metastatic disease. All patients were examined with a whole-body magnetic resonance imaging, or computed tomography, and positron emission tomography-computed tomography depending on the incorporation of the new technology in our medical institutions. They were studied according to the following procedure: 1) preoperative determination of regional lymph node along with the estimation and localization of sentinel lymph node (SLN) (dynamic isotope lymphography); 2) intraoperative localization and SLNB (lymphatic mapping); and 3) histopathology. Patient and tumor features were collected. RESULTS: Mean follow-up was 40 months, and 37% had a follow-up ≥ 5 years. A positive SLN was identified in 28 patients (56%). No significant difference in melanoma-specific overall survival was observed in terms of the primary tumor site. Hazard ratios (HRs) were statistically significant for SLNB-positive group and mitotic rate (MR) > 3 mm2, but not for presence of ulceration. Mortality risk in the SLN-positive group was almost fourfold greater than that in the SLN-negative group at any time of follow-up. CONCLUSIONS: SLN status, along with MR, can provide valuable prognostic information in patients with thick primary cutaneous melanoma.

Despite the growing interest in the potential electrochemical applications of both aluminium and ionic liquids in batteries, the microstructure of mixtures of trivalent salts and these dense ionic environments is completely unknown. In this work, the solvation of Al3+ cations in highly dense ionic solvents is investigated. For this purpose, molecular dynamics simulations of mixtures of a protic ionic liquid, ethylammonium nitrate (EAN), with aluminium nitrate (Al(NO3)3), both in bulk and confined between graphene walls, are performed. Several structural quantities of the system are calculated for different salt concentrations, such as densities, radial distribution functions, structure factors, coordination numbers and hydrogen bonds for the bulk mixture and ionic density profiles for the confined ones. Moreover, vibrational density of states is calculated for the salt cations, both in bulk and when close to the walls. The results obtained are analyzed and compared to those for mixtures of EAN with monovalent and divalent salts, in order to probe the influence of the salt cation charge on the system's properties. Finally, ab initio density functional theory calculations were performed in order to analyze the structure of the Al3+-ligand complexes, and their predictions for the Raman spectrum are compared both to the corresponding experimental one and the one coming from molecular dynamics simulations. According to our calculations, [Al(NO3)6]3- octahedral complexes do not significantly change the microstructure of the mixtures relative to those of Mg2+-based ones.

Resumen Introducción. Los sistemas de transporte urbano imponen barreras a las personas en situación de discapacidad, que les impiden desarrollar con normalidad actividades cotidianas como visitar al médico, estudiar o trabajar. Esta situación puede exacerbar un círculo vicioso en el que las personas en situación de discapacidad muestran menores niveles de formación, menores niveles de ingreso y, en general, menor bienestar. Objetivo. Hacer una caracterización de las personas en situación de discapacidad de la ciudad de Tunja, para conocer sus patrones de transporte y los problemas de accesibilidad al transporte público. Materiales y métodos. La investigación, localizada espacialmente en Tunja, se basó en una encuesta domiciliaria de viajes aplicada en el año 2012, que permitió conocer tanto las actividades económicas de los hogares como los patrones de viaje de la ciudad de Tunja, y una encuesta específica dirigida a las personas en situación de discapacidad, aplicada en el año 2014, que permitió identificar los principales problemas que deben enfrentar estas personas cuando viajan. Resultados. Se hizo una caracterización socioeconómica de la población en situación de discapacidad. Se encontró que el 95% de esta población tiene dificultades para transportarse en la ciudad, aunque el 88% de ellos usa el servicio de transporte público, especialmente taxis, lo que incrementa sus costos de transporte. Conclusiones. Se considera una primera contribución al desarrollo de una investigación empírica relacionada con los patrones de viaje y problemas a los que se enfrentan las personas en situación de discapacidad en la ciudad de Tunja, en Colombia.


Abstract Introduction. Urban transport systems impose barriers to physically impaired people, which impedes them from normally carrying out daily activities such as visiting the doctor, studying or working. This situation can exacerbate a vicious circle in which these people evidence lower levels of education and income and, in general, lower welfare. Objective. Perform a characterization of the physically impaired people in Tunja to know their commute patterns and problems of accessibility to public transport. Materials and methods. The research, located in Tunja, was based on two surveys: a household survey of transport applied in 2012, which allowed to know the households' economic activities as well as the commute patterns of the city of Tunja; and a specific survey conducted with physically impaired people in 2014, which enabled us to identify the main problems they must face when commuting. Results. A socioeconomic characterization of the physically impaired population in Tunja was made. It was found that 95% of this population have difficulties to commute in the city, and 88% use the public transportation service, especially taxis, which increases their transport costs. Conclusions. This work can be considered as a first contribution to the development of empirical research related to the commute patterns and problems of the physically impaired people in the city of Tunja, Colombia.

Objetivo. Fazer uma caracterização das pessoas em situação de deficiência física da cidade de Tunja, para conhecer seus padrões de transporte e os problemas de acessibilidade no transporte público. Materiais e métodos. A investigação, localizada espacialmente em Tunja, se baseou em uma enquete domiciliária de viagens aplicada no ano de 2012, que permitiu conhecer tanto as atividades económicas dos lares como os padrões de viagem da cidade de Tunja, e uma enquete específica dirigida às pessoas em situação de deficiência física, aplicada no ano de 2014, que permitiu identificar os principais problemas que devem enfrentar estas pessoas quando viajam. Resultados. Se fez uma caracterização socioeconómica da população em situação de deficiência física. Se encontrou que 95% desta população têm dificuldades para transportar-se na cidade, embora que 88% deles usa o serviço de transporte público, especialmente taxis, o que incrementa seus custos de transporte. Conclusões. Se considera uma primeira contribuição ao desenvolvimento de uma investigação empírica relacionada com os padrões de viagem e problemas aos que se enfrentam as pessoas em situação de deficiência física na cidade de Tunja, na Colômbia.
